Do You Qualify for Hospice Care?

Various diseases such as cancer, Alzheimers and Dementia, End Stage Diseases (Cardiac, Liver, Lung and Renal), HIV, Neurological conditions, etc., can qualify a patient for hospice. Hospice Care of the Lowcountry accepts all ages and offers specialized care for children. Patients can also consult their primary care physician to decide if hospice would be a beneficial option.

When should hospice start?
Once a person who is suffering from an advanced, life-limiting or terminal illness reaches the point at which treatments and curative measures are no longer a viable option. The sooner hospice care is initiated, the better, as it is designed to provide much-needed physical, emotional and spiritual support to the patient and their loved ones.
A patient’s team usually consists of the patient’s personal physician, hospice physician or medical director, nurses, aides, social workers, spiritual care counselors, bereavement professionals, speech, physical and/or occupational therapists, trained volunteers. This team is responsible for managing the patients symptoms, providing necessary medical supplies and equipment, coaching the family on how to care for the patient, and supporting the patient and family through their emotional, physical and spiritual needs, including bereavement counseling for loved ones.
The goal of hospice is to keep our patient out of the hospital and receive care in their residence. This allows the patient to remain in a comfortable, familiar environment, while still receiving personalized care from experienced professionals. Patients and their families have access to necessary medical supplies and medications for their care, covering physical, emotional and spiritual needs. Hospice allows the patient a sense of control over their end-of-life journey and provides support for loved ones throughout the entire process.
Yes. Receiving hospice care is always a choice. A patient may leave hospice and return to curative treatment if that is their choice.
If a person does not have any type of coverage, Hospice Care of the Lowcountry is guided by its mission to care for anyone in need, regardless of ability to pay. Hospice Care is covered 100% by Medicare, Medicaid, and most private insurance plans.
